- Application for Admission - Parent Questionnaire (Must contain student's Social Security Number)
- Parent Interview Form
- Application for Admission - Student Questionnaire (Grades 6-12 only)
- Parents (or legal guardian's) Statement of Agreement
- Family Financial Commitment
- Student Medical Information Form (Must be notarized)
- Student User Agreement for Technology Resources
- Application Fee
Required for Interview: Items necessary to schedule an interview with the principal.
- Church Reference Form
- Kindergarten Information - For Parent to Complete
- Kindergarten Information - For Preschool Teacher to Complete
- Original Birth Certificate from state of birth (not hospital copy)
Required for Admittance to Class: Items necessary for student to attend class.
- Kentucky Immunization Form - All students must have a current Immunization Certificate on file. This form may be obtained from your physician or health department (KRS 214.034)
- Physical Exam - Required for all Kindergarten, 6th grade and Transfer students (704 KAR 4:020)
- Eye Exam
Required for all Kindergarten students and must be performed by a Kentucky optometrist or ophthalmologist. (KRS 156.160)
